Avident Health LLC and Unite for HER have been awarded a quality improvement grant funded by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN) to enhance care for patients with metastatic breast cancer (mBC). The collaboration will use Avident Health's MyNavigator digital platform to deliver personalized education and connect patients with supportive services.
Metastatic breast cancer requires complex, ongoing treatment management, with adherence, symptom monitoring, and whole-person supportive care being critical. However, patients and their families often struggle to navigate clinical guidelines and access community resources outside of traditional appointments. This initiative aims to address those gaps by providing a home-based, interactive companion that translates NCCN Guidelines for Patients into accessible modules tailored to each patient's treatment journey.
The MyNavigator platform will also streamline access to Unite for HER's free integrative services, including nutrition counseling, fresh food delivery, mental health counseling, and evidence-based therapies. By integrating these services directly into daily navigation prompts, the platform seeks to remove barriers to supportive care. Additionally, the platform engages informal family caregivers by including them in a secure communication loop, ensuring they have the education and early symptom alerting needed to support treatment adherence at home. Patient-reported check-ins and symptom trends will be captured to alert clinical teams early, potentially preventing complications and reducing acute care utilization.

This initiative matters because it addresses the critical need for continuous support for metastatic breast cancer patients, potentially improving outcomes and quality of life while reducing the burden on clinical teams. By leveraging digital tools to bridge communication gaps, the collaboration could serve as a model for integrating supportive care into oncology practice, ultimately benefiting patients, caregivers, and the healthcare system.
