Emplify Health, a not-for-profit regional health system, and Homecare Hub, a home care and housing coordination platform, are collaborating to introduce Transitional Placement Services (TPS), a groundbreaking model aimed at improving the transition of patients from hospitals to community care settings. This initiative is set to launch in fall 2025, starting with a pilot program in La Crosse, Wisconsin, and is designed to tackle the critical gap in patient care exacerbated by the aging baby boomer population, often referred to as the 'silver tsunami.'
The partnership leverages Emplify Health's clinical expertise and Homecare Hub's technology platform to streamline the coordination of care for patients with complex needs. The TPS model involves a comprehensive evaluation by a multidisciplinary team, followed by the development of a personalized care plan and placement into a transitional Homecare Hub small home. This approach aims to ensure patients receive the appropriate care to facilitate an optimal discharge, whether to a long-term care facility, assisted living, or in-home care.
The initial launch in La Crosse will include six dedicated beds in a TPS home, with plans to expand based on demand and pilot data. This model not only promises enhanced patient care and reduced hospital readmissions but also aims to alleviate the burden on hospital staff and improve cost efficiency. The partnership is a testament to the power of innovation and collaboration in addressing some of the most pressing challenges in healthcare today.
